Sick (fit) notes

Patients can do a self certificate for the first 7 days. You do not need to see a GP.
Please find a copy of the self certificate form below.

After 7 days patients will require a sick note.

To request a sick note please fill out an online Triage Form requesting a sick note. If you are unable to complete this Online, please telephone Reception on 0161 493 9101 who will be able to help you.

If you are sick for more than 7 days your employer may ask you to provide medical evidence. This is to support payment for Statutory Sick Pay (SSP).

It is up to your employer to decide whether you are incapable of work. A medical certificate or Fit Note which is provided by your doctor is strong evidence that you are sick. A medical certificate will be accepted, unless there is evidence to prove otherwise.

You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist. Your employer will decide whether this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP.
